Special Considerations
Neonates (0-28 days)
Neonates require specialised assessment tools and should not routinely be assessed using standard PEWS:
- Consider using neonatal early warning scores (NEWS or Newborn EWS)
- Vital sign ranges differ significantly from older infants
- Signs of deterioration may be subtle (feeding difficulties, colour changes)
- Low threshold for escalation to neonatal team
Children with Chronic Conditions
Children with chronic conditions may have adapted to abnormal baselines:
- Cardiac conditions may affect normal heart rate and BP ranges
- Respiratory conditions may result in chronic oxygen dependence
- Neurological conditions may affect baseline consciousness levels
- Document individual patient's normal baseline parameters
- Look for change from baseline rather than absolute values
